To proceed with a VertaCat reservation request for rental, individuals must:

Step 2: Read all VertaCat User Rules & Guidelines and Complete the Personal Info / Accompanying Consent Form
Step 3: Complete Participant Liability Waiver and Release Form

Important: This waiver only needs to be completed once per participant to reserve adaptive golf equipment. After submitting, you will not need to fill it out again for future reservations.
